There are times when the information is important, or even crucial:
n When you’re at the Emergency Room.
n On admission to the hospital.
n During a visit to your physician.
n Getting a prescription filled at the drug store, because your pharmacist may be most likely to notice when medications will clash.
Now Shore Memorial Hospital is making it easier to keep track. Pharmacy Director Jim Davis has developed medication cards that can be filled out, folded and stashed in a wallet – for easy access any time, including emergencies.
It’s all part of a national healthcare safety movement. “The idea is that if a patient has that list of medications with them, every caregiver has a better idea of how to treat them,” Davis said.

Don't forget to SPEAK UP, because the most important member of your care team is you!
ASK about new medications prescribed or given in the
hospital. How will they help? What are the side effects? Are they safe to take with your other medications?
IN THE HOSPITAL, don’t accept any medication unless your nurse checks your armband to make sure it’s for you.
SPEAK UP if you have any questions, don’t feel right or suspect something may be wrong.

The information could be life-saving.
The list needs to be complete, including prescription drugs, over-the-counter medications, herbal remedies and even vitamins. “A lot of herbal drugs interact with regular drugs,” Davis said.
Patients should also make note of any allergies they may have.
